Quadruped Hold with Trunk Support

Purpose:

To improve core stability, shoulder strength, and postural control while supporting cervical extension and head control. Providing trunk support allows the child to maintain alignment and engage in sustained weight-bearing through the upper extremities with reduced fatigue. 

Directions:

Place a bolster, therapy roll, folded towel, or pillow under the child’s trunk. Have the child assume a quadruped position with hands and knees on the floor, shoulders stacked over wrists and hips over knees. Ensure the trunk support is positioned to assist alignment without restricting movement. Cue the child to gently lift the head and extend the neck while maintaining weight-bearing through both hands and knees. Hold the position for the recommended duration, then allow a rest break before repeating. 

Recommendations:  

  • Ensure even weight-bearing through both hands and knees. 

  • Adjust the height or firmness of the trunk support to optimize alignment. 

  • Monitor for compensations such as shoulder elevation, trunk sagging, or excessive lumbar arching. 

  • Encourage steady breathing during the hold. 

  • Progress by decreasing trunk support height or increasing hold duration as tolerated.
